Passport Acceptance Facility
Required Documents
All the following documentation is needed in order to be seen. All paperwork, photocopies and pictures must be completed prior to arriving at the appointment:
- Original Birth Certificate or original Certificate of Naturalization plus photocopy
- Valid Photo I.D. (For minors, school I.D. is acceptable) photocopy front and back.
- For minors 16 to 17 years age, must have one parent present.
- For children 15 and under both parents or the child's legal guardian must appear with child. If one parent cannot be present, one of the following documents must be provided:
- Court ordered papers stating full custody
- Form DS-3053 "Statement of Consent"
- Form DS-5525 "Statement of Exigent/Special Family Circumstance"
- Passport Application - completed with black ink pen only, but not signed. Applications available online at Travel.State.Gov
- Passport photo 2 by 2 inch
Federal Fees: Must be paid with a check or money order only and made payable to the U.S. Department of State
- Passport Book - $130 adults / $100 children: Good for all modes of transportation
- Passport Card - $30 adults / $15 children: Only good for land travel to Mexico and Canada and some smaller cruises
- Optional Expedited Shipping - $60
City of West Covina Fees: Payments can be made with cash, checks payable to THE CITY OF WEST COVINA or with a Visa or Mastercard.
- Processing Fee - $35
New passport applicants accepted only. Renewals can be mailed out on your own without authorized personnel. Children under the age of 16 must apply again as a new applicant.

Passports
As of June 1, 2009, All U.S. Citizens Traveling by Land or Sea Between the United States and Canada, Mexico Central and South America, the Caribbean and Bermuda, May Be Required to Present a Valid U.S. Passport Book or Card.
The fees are set by the federal government and are universally applied by all Passport Application Acceptance Service facilities. Fees per passport are as follows:
Passport Book - Routine Service (to Receive Passport Within Four to Six Weeks)
Children 15 and Under:
- Passport Book:
- $100 payable to U.S. Dept of State
- $35 payable to City of West Covina
- Passport Card:
- $15 payable to U.S. Dept of State
- $35 payable to City of West Covina
- Passport Book and Card:
- $115 payable to U.S. Dept of State
- $35 payable to City of West Covina
- Note: Both parents must appear with children 15 and under
Adults 16+:
- Passport Book:
- $130 payable to U.S. Dept of State
- $35 payable to City of West Covina
- Passport Card:
- $30 payable to U.S. Dept of State
- $35 payable to City of West Covina
- Passport Book and Card:
- $160 payable to U.S. Dept of State
- $35 payable to City of West Covina
- Expedited Service (to receive passport in approximately two to three weeks)
- The expedited service fee is an additional $60 added to the passport fee, payable to the "U.S. Department of State."
Items Required When You Apply For A Passport
Complete and submit application in person. Each application must be accompanied by all of the following:
- Proof of U.S. Citizenship - previous U.S. passport, certified copy of long-form (not "abstract") birth certificate or original Certificate of Naturalization or Certificate of Citizenship with foreign birth certificate for minors.
- Submit a Photocopy of each form of Identification documentation presented (i.e. Driver's license), showing both sides of ID on 8½ x 11paper.
- One passport photos.
- Appropriate fees - Payable by Check or Money Order Only.
- Both parents must appear for applicants 15 and under.
- Passport application - completed with black ink pen only, but not signed.
There are additional requirements for children 15 and under:
- Both parents or the child's legal guardian must appear with child and present valid identification.
- Must submit evidence of the child's relationship to parents / guardian(s) such as an original certified copy of birth certificate or adoption decree even if applying for subsequent passport.
- If only one parent is able to appear they must present either evidence of sole custody or second parent's notarized written consent to the issuance of a U.S. passport.
The U.S. Department of State now requires that the customer supply a photocopy of their identification at the time of passport application acceptance.
The City is not responsible for providing photocopies for passport applicants. As a courtesy, the City offers Photocopy Services for $0.25 per page. If international departure is in less than two weeks or passport is needed for foreign visas, it is advised that you apply at a Regional Passport Agency; an appointment is required.
